Authorization for Cremation and Disposition
The Cremation Authorization is an important legal document. Please read it carefully. You may print and sign this 4 page letter size pdf form and return it to us as soon as possible by fax at 636.227.7108, or by scanning the document, attaching it to an email and sending it to

Authorization for FLAMELESS Cremation and Disposition
The authorization is specifically for cremation by alkaline hydrolysis (water cremation or flameless cremation) only. It is an important legal document. Please read it carefully. You may print and sign this 4 page letter size pdf form and return it to us as soon as possible by fax at 636.227.7108, or by scanning the document, attaching it to an email and sending it to

Waiver of Right of Sepulcher or Disposition
This form may be required if someone other than the person with the legal right under Missouri law is directing the arrangements or disposition of the deceased.
Release of Remains to the Funeral Home
This form authorizes Schrader to remove the remains from the institution, medical examiner's office, or place of death to Schrader Funeral Home & Crematory. You may sign and fax to 636.227.7108 or email to
office@schrader.com. The completed form along with a clear copy of your driver's license or other government-issued ID is required.
Authorization for Embalming
This form authorizes Schrader to perform the embalming operation. You may sign and fax to 636.227.7108 or email to
